Vitamin D 1,25-Dihydroxy (calcitriol) is the biologically active form of vitamin D, produced primarily in the kidneys. Unlike the more commonly tested 25-Hydroxy form (which reflects vitamin D stores), calcitriol reflects how much active vitamin D your body is actually using. This test is essential for evaluating patients with kidney disease (where conversion to active vitamin D is impaired), hypercalcemia, hypocalcemia, sarcoidosis, and other granulomatous diseases. It is also useful when 25-Hydroxy vitamin D levels are normal but symptoms of vitamin D-related disease persist. The two forms of vitamin D should be interpreted together for a complete picture.
