Osteocalcin is a protein produced by osteoblasts (bone-forming cells) and is the most specific marker of bone formation. It reflects how actively your body is building new bone. Low osteocalcin indicates reduced bone formation and increased fracture risk, while elevated levels can indicate high bone turnover seen in Paget's disease, hyperparathyroidism, or bone metastases. Beyond bone health, osteocalcin is emerging as an important metabolic hormone linked to insulin sensitivity, glucose metabolism, and energy regulation. This test is valuable for monitoring osteoporosis treatment, assessing fracture risk in postmenopausal women, evaluating vitamin K status (osteocalcin requires vitamin K for activation), and tracking bone health in athletes and aging adults.
